COGNITIVE DISABILITIES STRATEGY Phone: (306)691-6459 455 Fairford Street E Mental Health & Addictions Services participates on this provincial intersectoral strategy that addresses extreme needs of those individuals with cognitive disabilities. Access Individuals, families or Case managers from community agencies may apply for assessment, consultation or benefits. Referrals and consent forms are forwarded to selected agency personnel from Mental Health & Addictions Services, Ministry of Social Services, Corrections and Public Safety, Prairie South School Division, or Holy Trinity School Division. Referrals are reviewed on a monthly basis by the South Central Cognitive Disabilities Strategy Intake Committee.
