Community Mental Health Nurses work with clients in the community and coordinate the range of services required by the clients in order to maximize the client’s level of functioning. A case management approach offers coordination of clinical and administration functions including assessment, counselling, planning, linkage, advocacy and monitoring. This includes a recovery focus involving training in basic living skills and crisis intervention, and vigorous outreach in order to assist clients in achieving their potential for an increased level of functioning in the least restrictive manner.
Other services include, but are not restricted to: medical and inpatient treatment, medication management, residential placements and monitoring, social and recreational programs, prevocational, vocational, and educational programs, family supports, and connections with community agencies and services.
The primary function of services is to collaborate with a community-based comprehensive range of recovery and support services to people who experience severe and persistent mental illness and/or addiction by an interdisciplinary team.

- Depot clinic - depot medication is provided to clients as prescribed by psychiatrist.
- Early psychosis intervention program support
- Cognitive disabilities strategy support
- Reintegration of long term detention clients from Saskatchewan Hospital, North Battleford
- Shared Management - a Community Mental Health Nurse provides clinical consultation to long term care facilities. Services include: assessment, review of client progress, introduction and review of behaviour management techniques, contacts with pertinent caregivers and documentation for the attending psychiatrist. Services are offered on site at Pioneer Lodge, Capilano Court, Extendicare, and Chez Nous in Moose Jaw.

Satellite clinics are provided in Assiniboia, Gravelbourg and Central Butte.
Discharge/Transfer or Completion
Community Mental Health Nurses are case managers and in cooperation with the client and caregivers assess and determine client need for programs, frequency of contact required and duration of services.
Rehabilitation Clients are discharged when they:
- move outside the Health Region, at which time transfer to another Region may be arranged;
- are incarcerated or hospitalized outside the Region for an extended period of time;
- placed in a special care facility. In this case, transfer to Shared Management Program would occur;
- refuse services, is non compliant and does not experience psychotic episodes; or
- have been stable for an extended period of time and maintain adequate supports in the community.
