The complex needs process is intended to provide effective human services that contribute to the well being of people with complex needs. This process usually involves several human services agencies and leads to a plan that promotes access to services to address the needs of the most vulnerable, building on the strengths of individuals, families and communities.

Access

Mental health and/or addictions professionals identify cases for review by a complex case committee.  Criteria for complex case are well defined and targeted to the individual with a range of needs and required supports that have been unable to be addressed through current programs and services in the Region.

Discharge/Transfer of Completion

Client need determines the intensity and frequency of services required. Referrals to appropriate services are made in accordance with the client’s community support plan.
