This initiative is intended to support the acute care system by improving the capacity for early hospital discharge, to avoid/prevent re-admission and to avoid/prevent imminent admission.

A Community Mental Health Nurse provides assessment, family support, and identifies and coordinates at- home supports.   A Home Service Worker provides direct services as determined by the needs assessment.

Access

Services are provided on a part time basis Monday through Friday.

Referrals for service will be accepted from the inpatient unit for clients meeting at least one of the following criteria:

  • hospital stay of 21 days;
  • psychiatric symptoms can be managed with supports outside of the hospital;
  • readiness for discharge;
  • ability of client and family to benefit from brief in home  and community support.

Exclusion Criteria

Referrals for service will not be accepted if the client demonstrates any of the following:

  • represents an imminent threat to self or others
  • cannot be treated safely in care outside of the home
  • does not show an ability to care for physical and personal needs

Discharge/Transfer of Completion

Clients who require services on a continuing basis will be referred to the appropriate service area.
